SUBSTITUTE REQUIREMENTS CHECKLIST:

18 years of age or more
o I9 Form and Documentation
Valid substitute certificate or teaching certificate
IVP Level Fingerprint clearance card
HS Diploma or official transcripts indicating completion of 30 credit hours
MMR Record/Vaccine Record

New Substitute Teachers: Upload employment documents to ESI by uploading them at https://www.educationalservicesinc.com/employee-resources/document-upload or emailing them to documents@esiaz.us, or texting to 480-719-3271.

Substitutes already cleared by ESI: Contact Human Resources at hrdept@ssd11.org for an appointment to activate District email address and conduct District Orientation
